This is a homework log sheet that my students use when they come to class with an incomplete homework assignment. It puts all the responsibility of the "missing" homework on the student. It stays in my classroom (in the student portfolios) and when students come without their homework completed, they fill it in for the assignment they are missing while I collect it from the students who are prepared.
